On behalf of myself and my child(ren) named above, I hereby acknowledge and agree to the following terms and conditions in consideration of our ability to participate in the 2025 Youth of the Year Rally organized by the Boys & Girls Club of the Foothills:
Assumption of Risk: I understand and acknowledge that there are inherent risks associated with participation in the activities planned for the 2025 Youth of the Year Rally. I further acknowledge that we are voluntarily participating in the activities, which I agree are dangerous and entail both known and unknown inherent risks, including the risk of injury, permanent disability, or even death, deriving from, but not limited to, equipment malfunctions; design defects; lack of supervision and/or trained monitors; lack of proper equipment or padding, netting, or other safety measures; slipping; falling; landing; or colliding with fixed objects or other people, as well as the negligence and/or omissions committed by me and/or my child(ren). I voluntarily assume all such risks on behalf of me and/or my child(ren).
Release of Liability: I hereby expressly, unconditionally and voluntarily remise, release, waive, discharge, and agree and covenant not to sue Boys & Girls Club of the Foothills, its directors, officers, employees, volunteers, agents, and representatives and agree to hold said parties harmless from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child(ren) or myself while participating in the 2025 Youth of the Year Rally event, whether caused by the negligence or omissions of the Boys & Girls Club of the Foothills or otherwise.
Parent Responsibility: I understand I am required to supervise my child for the duration of our time at the event and that the staff of Boys & Girls Club of the Foothills are not providing child care at this event.
Indemnification: I agree to indemnify and hold harmless the Boys & Girls Club of the Foothills, its directors, officers, employees, volunteers, agents, and representatives from any and all liabilities, damages, costs, or expenses, including attorney fees, arising out of or related to my child(ren)’s participation in the 2025 Youth of the Year Rally event.
Photography and Video Release: I grant the Boys & Girls Club of the Foothills permission to take photographs and videos of my child(ren) during the 2025 Youth of the Year Rally event for promotional and marketing purposes without compensation to me or my child(ren). I understand that these photographs and videos may be used in print publications, online publications, presentations, websites, and social media.
